B/X says rest one turn in six, I've yet to meet a DM that invokes that rule.
I agree that this rule is likely one of the less invoked rules of game... and I purposely inserted a similar one in my set of rules... the funny aspect of my chioce is that I have never used that rule since a fight in my game (that's where fatigue impacts more of course!) lasts max 3 rounds...
